1. ALL APPLICANTS

Documentation to prove residency at the address stated on your application, and that you will be resident there for 6 months or more.

For Example: Utility bill, council tax, landlord statement etc naming you as householder.  

(Not mobile phone or bank statement)

To register children under the age of 16 yrs, one or more parent/legal guardian must be registered with the Practice.

2.  CURRENT NHS PATIENTS

We will also need to see and copy one of the following documents when you apply to register with us.

NHS medical card.

A passport showing that the holder is a British citizen, or has a right of abode in the United Kingdom.

3.  NON NHS PATIENTS

We will also need to see and copy the following documents when you apply to register with us.

A passport showing that the holder is an EEA citizen

Any other Non EEA Passport

PLUS

A visa showing that the holder can stay in the United Kingdom for twelve months or more from the date of application.
