The quickest way to contact the surgery for all clinical and administrative queries including to book an appointment is via our online contact form.

Medical request forms are available Monday-Friday, 8am-2pm.

The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays.  They will not be read Monday-Friday between the hours of 18:00 – 08:00. Please contact 111 during out of hours or 999 if this is an emergency.

 

WEDNESDAY 8TH OCTOBER 2025

Dear Patients, we have turned our triage form off this morning as we have multiple GP’s off sick, if we can turn triage back on today we will do so. Please accept our apologies for this.

If you need urgent attention that cannot wait please contact Hawthorne Walk in Centre on 0161 220 6080, you can also complete (their) online form via their website, alternatively call 111 for advice.
